Petition to Review an agreement for payment of compensation is used to correct mistakes

  1. A Petition to Review an Agreement for Compensation or a Notice of Compensation Payable (NCP) to correct mistakes is used when the employer or the employee believes that a material mistake was made at the time the original agreement was made.

    The Petition to Review is also used to add or modify the Agreement or NCP where there has been a change in the nature of the injury at some time subsequent to the issuance of those documents.

    Among other things, the Petition may be used by the employer to do the following:

    Correct a mistake in the description of the injury
  2. Correct a mistake in the average weekly wage calculation
  3. Review the medical treatment or billing, or
  4. Correct the date of injury.
  5. The employee may use the Petition to do the following:

    Correct a mistake in the description of the injury
  6. Modify or expand the description of the injury
  7. Review medical treatment or billing
  8. Challenge a benefit offset
  9. Correct a mistake in the average weekly wage calculation, or
  10. Correct the date of injury.
