Employers are obligated to file various notices under the Pennsylvania Workers’ Compensation Act
- In addition to the employee required reporting forms under the changes to the Workers’ Compensation Act in 1996, commonly known as Act 57, employers are required to file appropriate forms in the following circumstances:
LIBC Form 764: Notice of Change of Workers’ Compensation Disability Status
- LIBC Form 757: Employer Notice of Ability to Return to Work
- LIBC Form 751: Employer Notice of Suspension/Modification on Return to Work
- LIBC Form 502: Employer Notice Stopping Temporary Compensation Payable
- Panel Provider Notification
